About the Role
We are seeking a compassionate and driven Case Manager to coordinate and oversee the delivery of high-quality, individualised home care services.
In this role, you will ensure care plans are flexible, client-focused, and aligned with current Home Care legislation. You will build strong, lasting relationships with clients, families, healthcare professionals, and external stakeholders to deliver safe, supportive care aligned with client goals and Sunny Care’s philosophy of independence, flexibility, and choice.
This is a maternity leave cover position, with strong potential for ongoing employment based on performance.
Location: Gold Coast (with occasional travel to the Brisbane office for meetings and events)
About You
We’re looking for a respectful, empathetic professional who thrives on building relationships and delivering exceptional care.
You will bring:
Bilingual in Chinese (Mandarin and/or Cantonese) and English, with strong verbal and written communication skills
Australian working rights (citizenship or permanent residency preferred)
Minimum 1 years’ experience in Sales, Business Development, Case Management, or Healthcare
Experience in aged care or community services (CHSP, HCP, or Support at Home highly regarded)
Strong interpersonal and relationship management skills with a client-centred approach
Understanding of service coordination, client onboarding, and basic scheduling processes
Proficiency in client management systems and Microsoft Office
Valid driver’s licence
National Police Check (or willingness to obtain)
